New York Avenue, Brooklyn, New York; as Rachel SLOMSKY, 62; born 5 Jun 1881 in Wisoke; father Hershel KYIACK, mother Sarah GOLD, husband Joseph; buried 11 Jun 1943 in Mount Hebron Cemetery; headstone has Riszka daughter of Tzvi Tuvia.
